School Enrollment Health Requirements

All students who are entering Pre-K, Kindergarten, 6th and 9th grades are required to have updated immunizations and a physical completed upon entering the 2010-2011 school year. Additionally, effective January 1,2008, children enrolling in Kindergarten are required to have an eye examination by a licensed eye doctor or optometrist. All Illinois children in Kindergarten, 2nd and 6th grade are required to have an oral health examination. Each child registering in the above specified grades must submit proof of examination.

This year,School District 168 is teaming with Advocate Hope Children’s Hospital to provide opportunities to have your child’s physical and immunizations completed. The Ronald McDonald care mobile will be coming to Wagoner School 1831 E 215th Pl, Sauk Village on August 16. 2010 from 9am-2pm, space is limited therefore students will be seen by appointment only, unfortunately we cannot accept children with private/commercial insurance. Information will be going home to parents, any additional questions please contact your child’s school and ask for the nurse.


School Health Policies

Medication Policy: The administration of prescription  medication by school personnel to students shall be done only with written authorization of parents and direction of a licensed medical physician/dentist. Prescription medication must be in the original containers with the pharmacy label attached. Ask the Pharmacist for a second bottle with label for school use.A doctors signature is not required for over the counter medications such as Tylenol, Ibuprofen, cold/cough medicine or Benadryl for allergies but they must also be in the original containers.Parents should bring their childs medication to the schools health office or main office.As always we encourage medication to be given outside of school times however we realize this is not always possible. Please help us by completing the necessary paperwork when your child does need medicine at school.

Smile Dental Program: Parents please watch for the Dental forms that will be coming home soon,  the mobile dentist will be coming to Wagoner school on November 29,2010 and Strassburg school on December 13,2010 this program is a highly acclaimed on-site dental care program created in compliance with the Center for Disease Control guidelines. The mobile dentist team will provide the following preventive services for our students, Dental exams, cleanings, Fluoride treatments and sealants a dental report card and a free toothbrush will be provided.

Screenings: Parents there are several screenings that are completed during the school year, they include Vision and Hearing , Height and weight and scoliosis screenings. When the screenings are completed parents and guardians will be notified of the results.

Parents and students often wonder whether they are too sick to come to school or if they should see a doctor, fortunately most viral illnesses have mild sighns and symptoms.

Colds: Colds start with sneezing, a runny nose, feeling tired and not well, sometimes a cough, sore throat and a low grade fever will also be present. Colds usually can last about 7 to 10 days Tylenol, Ibuprofen and plenty of fluids should help.

Seasonal Flu: Fever, cough, sore throat, stuffynose, headaches, bodyaches, chills and fever . Stay home if you have a fever over 100 degrees, are vomiting or have diarrhea.
